Overview
Thank you all for your interest in AYSO Region 35 (Cupertino) Soccer.
Pre-registration for PLAYERS and VOLUNTEERS is on WWW.EAYSO.ORG. Print both the Player and the Volunteer forms.

NOTE: For late registrants, once the WALK-IN dates are over, we may post mail-in registration on this web page if available.
Registration for the Fall season, runs through the summer months, and the season gets underway in mid-August. For the (slightly shorter) Spring season, registration runs through January and the season gets underway in mid-February. See our Seasons page for more details, or watch our Calendar.

Step 1. Pre-Registration
All players need to pre-register on-line at the eAYSO website, login required with email and password. PRINT OUT and BRING the signed registration and volunteer forms generated via that process, along with proof of age and fees to a walk-in registration session. We are AYSO Cupertino Region 35. Returning players do not need to bring proof of age.
Please fill in and print your volunteer form. No experience necessary and the league will provide all training. If you have no preference, you may sign up as a stand-by or backup volunteer. Questions? Please see our Volunteers page.
Payment: Cash, Money Orders or Checks -- Payable to AYSO 35 Cupertino. Please bring exact cash, we have no change.
Step 2. Walk-in Registration Dates
Please bring the printed registration forms, proof-of-age document (new players only), and payment, on one of these walk-in dates. Volunteer forms are also accepted at the same time.

- Player Address and Telephone Number
- Emergency Contact Name and Telephone Number
- Physician Name and Telephone Number
- Player's Gender, Birth Date and Age
- Medical Insurance Carrier Name
- Name of siblings in same division (will be on same team)
- Mother/Guardian Name and Home Telephone Number
- Father/Guardian Name and Home Telephone Number
- Information on any disabilities, handicaps etc.
- Parent/Guardian Signature and Date
- Parent/Guardian's Volunteer Form and Preference (if no preference, be a stand-by.)
