AYSO registered players, coaches, referees and volunteers, may be covered for accidental bodily injured while participating in AYSO related activities, under the conditions specified in the AYSO Soccer Insurance Policy Brochure.

You will need to fill and send in a Claim Form, with all relevant supporting documents.

Whether or not you need to take advantage of this coverage, all injury, incident or property damage events should be reported to the Regional Commissioner or Safety Director, using this Incident Report Form.
